Survey Terms and Conditions

1. By participating in this online survey, you agree to be bound by these terms and conditions.
2. In these terms and conditions, “we”, “us”, “our” and “GCNA” all mean Goodwill of Central and Northern Arizona.
3. Anyone who is a resident of Arizona and over the age of 18 may submit a single survey. Any survey must be fully completed to be considered for the promotion.
4. The survey will be completed digitally via a link.
5. There is no charge to submit a survey, though if entry is made digitally, data and messaging rates may apply.
6. Five (5) participants who submit a completed survey will receive a $50.00 Visa gift card determined through a random drawing. Winners will be emailed at the email address provided by the participant on the survey or notified via phone at the phone number provided by the participant on the survey. The Visa card will be delivered once contact is made with the winning participants. Each price is non-transferable, non-refundable and no cash alternative is available. GCNA may vary or swap the gift cards at its sole discretion, if it considers it necessary and appropriate in its sole discretion.
7. GCNA is not responsible for any survey lost, damaged or delayed as a result of any network, computer hardware or software failure of any kind, including faulty survey links.
8. If we consider that you or your entry are in breach of these terms and conditions, or if we have grounds to suspect any entrant is cheating, or engaging in deception or fraudulent conduct of any kind (including, without limitation, manipulation of the promotion), we reserve the right in our sole discretion (and without us incurring any liability) to disqualify any entry or person we reasonably believe to be responsible for, or associated with, such activity.
9. By entering the promotion, you agree to release us from any liability whatsoever for any claims, costs, injuries, losses, or damages of any kind arising out of or related to the promotion, or with the acceptance, possession, or use of the Visa gift card. In no event will our total aggregate liability to you exceed the value of the gift card.
10. The promotion, and these terms and conditions, are governed by the laws of the State of Arizona, and any disputes arising out of or relating to this promotion shall be subject to the exclusive jurisdiction of the courts sitting in Maricopa County, Arizona.
